The Chronic Pain Care Coordinator assists in coordinating appointments for pain management as well as coordinating specialty appointments such as;
- Orthopedics
- Neurosurgery
- Physical or Occupational Therapies
- Pain Clinic
- Mental Health Counseling
The Chronic Pain Care Coordinator also provides the following services;
- Serves as point of contact between patient and Provider for any questions or communication regarding the plan of care for chronic pain management
- Provides information related to pain medications, side effects and disease processes
- Assists in arranging alternate non-pharmacological treatment modalities such as dry needling, auricular acupuncture, massage or chiropractic services
- Assists patients in managing pain, optimizing function and maximizing quality of life with improved physical and psychosocial function
