PCHC is open to the public and contracts with many Commercial plans, Affordable Care Act plans, Medicare and Medicaid plans.
Medicaid HMO Plans
For all Wisconsin Medicaid HMO plans not listed below, we ask that you consult your insurance plan directory or call your insurance company's customer service department to find an in-network provider.
PCHC participates in the Wisconsin Medicaid/BadgerCare Plus program.
Out-of-state Medicaid is not accepted.

Anthem Blue Cross Blue Shield Medicaid HMO
Family Care Plans:
PCHC limits the contracted plans to Security Health Plan (SHP) and Care Wisconsin for Medicare Advantage. For all other Medicare Advantage plans please consult with your insurance carrier to determine your level of benefits for services at PCHC.
PCHC requires copays up front at the time of service. Your insurance company may require payment for part of your services. Be prepared to make a copayment when you check in for an appointment. You may be asked to pay for non-covered services in full before care is provided. For questions about copay, coinsurance, deductible or non-covered services, contact your insurance company.
Peter Christensen Health Center submits claims to primary insurance and, if applicable, secondary insurance, including Medicare and Medicaid. Patients receive a bill when an amount is due and payable. The patient is responsible for the amount in the “AMOUNT DUE” box. Balance due is payable when the statement is received.
For your convenience, payments can be made by cash, check or money order; credit cards (Visa, MasterCard, Discover, American Express) We also offer a courtesy payroll deduction for Tribal, LOTC and BDC employees.
*No one will be denied access to services due to inability to pay; and there is a discounted/sliding fee schedule available based on family sized and income.*
